Performance Ratings and Health Data Integration: An Update

Pharmacist and Technician Objectives:
At the conclusion of the program, successful participants should be able to:

1.  Review most recent pharmacy STAR rating cut-point changes.
2.  Discuss opportunities that exist for pharmacy to contribute to improving performance ratings for health plans.
3.  Discuss opportunities that exist for pharmacy to contribute to improving patient health outcomes in all practice settings.
4.  Recognize technology integrations arrangements for health data.
5.  Identify population health statistics that justify expansion of clinical integration strategies.
6.  Recognize existing models of health data integration.
 



This program is sponsored by the Alabama Pharmacy Association Research and Education Foundation (APAREF).  The APAREF is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.

CPE Credit: In order to receive credit for the programs you must provide APA with your NABP e-Profile ID as well as the month and day of your date of birth.  In addition, you must sign-in at the beginning of the program and complete an evaluation form at the end of the program.  No exceptions.  Your credit will be uploaded to CPE Monitor within four weeks of the program.  You must provide APA with all required information within 60 days of the program to receive credit.  APA cannot award CE credit after 60 days.

Registration Information:
APA Member Rate: $20 per person (includes dinner), $25 after early-bird pricing ends
Non-Member Rate: $32 per person (includes dinner), $37 after early-bird pricing ends
 
Refunds: 
If you are unable to attend the program, refunds will be made through the dates listed below for each regional meeting.  APA requires a written request for the refund.  No refunds will be made after the dates shown below for each program. Refunds will not be given for inclement weather.
